Key Responsibilities
- Program Provision: Facilitate and evaluate a variety of social support programs, including community and outing-based activities.
- Client Support: Use a client-centred approach, assisting clients with activities, daily living needs, and communication, while observing and supporting their well-being.
- Transport: Organise client transport to and from the venue, including use of personal vehicles and community buses as needed.
- Meals: Coordinate meal preparation and service, ensuring safety and quality standards.
- Continuous Improvement: Participate in improvement initiatives, attending relevant training and team meetings.
- Occupational Health & Safety: Support a safe environment for clients and staff by adhering to safety policies and performing safety audits as required.
About You
- Certificate IV in Leisure and Health, Ageing Support, Community Services, or equivalent
- Experience working with elderly clients and individuals with disabilities in a community care setting
- Strong understanding of client-centred programming, CHSP, and HACC PYP programs
- Knowledge of Food Safety and Occupational Health & Safety standards
- Strong communication skills and ability to work collaboratively in a team
- Basic computer literacy in Microsoft Office applications
- Valid Victorian Driver's Licence, reliable personal vehicle (with comprehensive insurance), and ability to drive a Council's 12-seater bus
- Current First Aid and Food Handling Certificates (or willingness to obtain)
- Current Police Check and Working with Children's Check
- Bilingual skills in a community language are highly desirable
