Occupational Health Nurse

Step into a rewarding role providing Occupational Therapy services to older adults within the Knox municipality under the My Aged Care Commonwealth Home Support Programme (CHSP). Work within a Wellness and Reablement approach to empower clients in maintaining independence and safety at home and in the community.

• Competitive pay: $60-$65 per hour

• 22.5 hours per week

• 1-month contract with potential to extend

• Deliver home-based clinical OT assessments and interventions for My Aged Care (MAC) clients referred by ACAS and RAS

• Assess clients' functional ability, home environments, and suitability for home modifications or assistive equipment

• Develop, implement, and review person-centred, goal-directed care plans

• Ensure compliance with audit and accreditation processes, and contribute to continuous improvement efforts

• Provide specialist OT advice, maintaining adherence to regulations, policies, and quality standards

• Collaborate with the Allied Health and Community Access & Support teams to deliver effective client-centered services

• Communicate with internal and external stakeholders, ensuring high standards of service provision

• Support the Allied Health Administration Officer with intake tasks and daily operations

• Bachelor of Occupational Therapy or equivalent qualification

• Minimum of three years' clinical experience as a Grade 2 Occupational Therapist

• Strong knowledge of My Aged Care CHSP and the Wellness & Reablement approach

• Experience engaging clients holistically to develop meaningful care plans

• Expertise in assessing and prescribing home modifications and assistive equipment

• Ability to work autonomously while collaborating effectively within a team environment

• Proficiency in client management software, technology, and standard office applications

• Strong time management, prioritisation, and organisational skills

• Exceptional interpersonal skills with the ability to liaise with diverse client groups and stakeholders
