Key Responsibilities
Deliver home-based clinical OT assessments and interventions for My Aged Care (MAC) clients referred by ACAS and RAS
Assess clients' functional ability, home environments, and suitability for home modifications or assistive equipment
Develop, implement, and review person-centred, goal-directed care plans
Ensure compliance with audit and accreditation processes, and contribute to continuous improvement efforts
Provide specialist OT advice, maintaining adherence to regulations, policies, and quality standards
Collaborate with the Allied Health and Community Access & Support teams to deliver effective client-centered services
Communicate with internal and external stakeholders, ensuring high standards of service provision
Support the Allied Health Administration Officer with intake tasks and daily operations
About You
Bachelor of Occupational Therapy or equivalent qualification
Minimum of three years' clinical experience as a Grade 2 Occupational Therapist
Strong knowledge of My Aged Care CHSP and the Wellness & Reablement approach
Experience engaging clients holistically to develop meaningful care plans
Expertise in assessing and prescribing home modifications and assistive equipment
Ability to work autonomously while collaborating effectively within a team environment
Proficiency in client management software, technology, and standard office applications
Strong time management, prioritisation, and organisational skills
Exceptional interpersonal skills with the ability to liaise with diverse client groups and stakeholders
