Working with one of Australia's leading providers of aged care and disability support services, delivering person-centred care to more than 4,500 clients with the help of 2,000+ support workers. They specialise in home care packages, NDIS services, supported independent living, dementia care, and allied health.
This is a remote position looking after clients located in Acacia Ridge QLD
About the Role
As a Senior Case Manager, you'll manage your own caseload of clients while providing leadership, guidance, and support to Case Managers. You'll oversee complex cases, mentor team members, and ensure services are delivered to the highest standards of quality and compliance. This is a key leadership role, balancing client care with workforce capability and operational support.
What you'll be doing:
- Manage a caseload, developing and reviewing individual care plans with clients and families
- Act as the point of escalation for complex client issues and complaints
- Mentor, guide, and support Case Managers to build capability and performance
- Oversee compliance with sector regulations, audits, and reporting requirements
- Contribute to rostering, staff resourcing, and quality improvement initiatives
- Build and maintain relationships with clients, families, suppliers, and community organisations
What you bring:
- Qualifications in Social Work, Community Services, Nursing, or a related field
- Strong experience in case management within aged care, disability, or community services
- Demonstrated leadership, mentoring, or supervisory experience
- Knowledge of Home Care Packages (HCP), NDIS, and aged care frameworks
- Excellent communication, problem-solving, and conflict resolution skills
- Ability to manage competing priorities in a fast-paced environment
